 This application form confirms my intention to enter the 2012 Raft Race having read the Conditions of Entry, I agree to abide by such Rules, Regulations and Safety Instructions and will undertake to confirm the amount of sponsorship we have pledged (£100 minimum) prior to the race commencing.

 All competitors under the age of 16 will produce written parental/guardian consent to their taking part in this event.

 I accept that I, and those for whom I am responsible, are taking part at our own risk.
